Question: Is the deep breath something that was planned, or George added it after-the-fact?

The breathing sounds like it was done on purpose unless he was full of the cold.

It was done on purpose, planned and recorded so it would sound like that,in fact, it took a lot of preparing. Paul McCartney has spoken of it and recalls watching John sing it.

I always think John has a cold on Drive My Car. And of course he definitely does on the Please Please Me album!

You can say that again!! That's just another incredible thing about them and their story--John's snout sounds completely stuffed, and they made an incredible record regardless. And maybe, just maybe, it gave him a grittier, harder vocal on 'Twist and Shout' than he otherwise would have had.
