I have Mike's book, 'Remember' that EnchantingLennon mentions here. It is a full page b&w photo, but sadly I have no web site and no way of posting it here.
John is standing to his right (our left) looking like a Goon, Pete on the other side just looking dopey and George is arms's length away, with his forefinger about 1 inch from Paul's nipple and staring seriously at it. Yours is the exact photo, just with bits taken out.

Actually just noticed caption: "One of my favorites because it absolutely personifies John. This is exactly what he was like (with Our Kid looking remarkably like Sylvester Stallone). This is one of the first 'wacky' Beatle pictures, very influenced by the Goons; Peter Sellers and Spike Milliagan were an important part of our upbringing, which is one reason the boys were keen to work with George Martin, who had produced comedy records for the Goons. This photo is a forerunner of the sort of stuff they did in A Hard Day's Night, directed by Dick Lester, who made the classic Running, Jumping and Standing Sill Film with Milligan. This is after a gig at the Tower Ballroom. "Rambo" Paul is rubbing down, and John's hair is plastered against hs skull with sweat."
